The principles of Wraparound include family voice and choice: the family’s needs are priority. The program is team based and revolves around each area working collaboratively to share the responsibility for the child. Wraparound is community-based and culturally competent working towards goals until no longer required.
A service provider, community member, or parent identifies a child or family in need and completes a referral.
The referral is reviewed by the statewide access site and the family is then contacted to have the Wraparound applications completed.
Once the family has completed all documents and selected an agency/provider the statewide representative will then submit the Wraparound application to DMHA/FSSA.
Upon approval, the selected agency and wraparound provider is notified to begin outreach to the youth and their family.
If the youth is denied or the family declines services, the family is referred to the access site for their county of residence for alternative options.
The family gets started by identifying their strengths, challenges, and family team.
• Families and children looking for support.
• Youth with two or more diagnoses.
• Insured and uninsured youth.
• Youth 6-17 years of age.
• “Traditional” services do not meet the family’s needs.
• Youth at risk of being removed from home.
The Wraparound Services team believed that “families don’t fail, plans do.” That’s why they work diligently with parents and guardians to ensure effective goal setting and success. By supporting open communication, sharing your family’s strengths and challenges, meeting regularly with your team, and engaging in the treatment plan, parents can be the most powerful advocates for their children. Often, team members are neither parent nor guardian but have a significant stake and impact on the family or child’s well-being. Once established as a team member, these individuals can attend meetings, support open communication, and share ideas or suggestions with the team.
A Wraparound Facilitator is trained to serve families with trauma informed and culturally competent care. They help identify a family’s strengths, challenges, and support structures. Facilitators schedule and lead team meetings facilitate communication and coordination of care, and monitor goals and successes for the families. Guided by Wraparound Principles, Wraparound Facilitators value and respect the family voice and choice.
