Licensed Practical Nurse

Provides direct
physical and mental health care of patients on the inpatient unit. Functions
as a part of the interdisciplinary treatment team under the direction
of the Registered Nurse.
| Essential Duties/Responsibilities |
To perform this

- Performs duties
under the direction of a Registered Nurse.
- Assists with
admission and discharge of patients under the direction of a Registered
Nurse. and according to inpatient unit procedure and Standards of Care.
- Administers medications
to patients following established procedures and Standards of Care
- Ensures that
the medication listed on the Medication Administration Record (MAR),
corresponds correctly to the licensed independent practitioner’s
(LIP) order.
- Ensures that
each medication ordered is transcribed correctly.
- Ensures that
the next shift has all necessary medications and supplies for each patient.
- Ensures medications
are appropriately procured in a cost-effective manner for each patient.
- Participates
as a member of the interdisciplinary team by carrying out treatment
interventions within limits of training and licensure and by communicating
observations regarding patients to other members of the treatment team.
- Documents all
care provided and patient response in the patient's medical record per
established procedures and Standards of Care on assigned patient caseload.
- Conducts patient/family
education related to medications, and general physical and mental health
needs.
- Participates
in Quality Improvement activities as assigned.
- Maintains professional
behavior consistent with the principles of customer relations, total
quality management, and teamwork.
- Maintains confidentiality
in accordance with established policy and procedure.
- Participates
in unit staffing meetings and continuing education activities to continually
update current knowledge regarding nursing in general and psychiatric
nursing in particular.
- Maintains basic
knowledge of computer software utilized for patient recordkeeping.
- Performs other
duties as assigned.
| Minimum Qualifications/Requirements |
- Knowledge and
skills regarding basic nursing care and general knowledge regarding
preparation and administration of medications.
- Good oral and
written communication skills.
- Basic computer
skills
| Certificates, Licenses, Registrations |
- Licensed by the
State of Indiana as a Licensed Practical Nurse.
- Maintain current
CPI and American Red Cross CPR/1st Aid certification.

- Exposure to disturbed
and potentially aggressive patients is possible. Stress ranges from
low to intense depending on situation.
- Physical strength
necessary to implement CPI interventions.
- Sitting, standing,
reaching, bending, stooping as necessary to provide individualized patient
care.
Environment is a
hospital-like atmosphere with access to an outside patio. May have to
help bathe and feed patients. This position requires the ability to work
under fluctuating levels of stress in a model of least restrictive care
and proactive quality customer service.
- Completion of
tuberculin screening no later than three days prior to first day of
employment and annually thereafter
- Completion of
a pre-employment drug screening and completion of post-employment drug
or alcohol tests upon reasonable suspicion of use.
- Completion of
Center-wide orientation and ALL required paperwork prior to reporting
for work
- Demonstrated
computer literacy through successful completion of pre-employment testing
may be required.
- Completion of
Total Quality Management training
- Attendance at
all mandatory staff development and training
- Successful completion
of a six month on the job orientation period
- Participation
in payroll electronic deposit
- Adherence to
Compliance Program Plan
- Satisfactory
reference and background investigation checks.
- Satisfactory
completion of an Indiana Department of Child Services criminal fingerprinting
background check at the time of hire and every four years (or at contract
renewal when applicable) for any employee that has direct contact on
a regular and continuing basis with DCS and IVB clients.
