Community-Based Nurse

Provides general
and specialized psychiatric nursing services to clients individually or
in groups. Primary activities include: train for and over-see medication
administration; monitor sites and program activities for safety, infection
control and disease prevention; conduct patient education and training;
and provide nursing assessments. Additional major duties include staff
training, report writing, serving on treatment teams, and on-call crisis
assistance.
| Essential Duties/Responsibilities |
To perform this
job successfully, an individual must be able to perform each essential

- Monitors and
coordinates basic health services and mental health services for designated
clients. Identifies client needs in all nursing-related areas, and initiates
and maintains effective programs and services to respond to those identified
needs.
- Serves as program
liaison with the clients’ various physicians. Obtains and correctly
transcribes physician orders. Calls in new physician’s orders
to the pharmacy provider. Monitors that physicians’ orders are
signed and being implemented. Notifies appropriate physician of any
client health changes or problems.
- Completes initial
and periodic nursing assessments on designated clients.
- Maintains an
adequate supply of medication for designated clients. Reorders medications
from the pharmacy provider. Monitors the security and safe storage of
all medications on Center premises. Maintains records and reports accounting
for all medications and their administration and/or other disposal.
Trains clients to self-medicate properly, and trains staff to administer,
monitor and store medications.
- Identifies needs
for patient education and direct training activities, especially related
to mental illness symptoms, health and fitness, medications, and medical
care.
- Monitors and
documents that clients are receiving all ordered: medical evaluations,
laboratory tests, allied health evaluations, and immunizations. Maintains
a routine of regularly recording clients’ weights and vital signs.
- Provides an injection
clinic for clients and staff, including but not limited to Hepatitis
B inoculations and annual PPD tests.
- Monitors premises
and program activities for infections, infestations, unsanitary conditions,
and safety risks. Reports problems as directed, and provides guidance
for remedial steps.
- Sees that all
physician orders for special diets, activity restrictions, and special
treatments are communicated and implemented.
- Serves as a resource
to the organization in the area of specialty for training, problem-solving,
and decision making.
- Performs other
duties as assigned.
| Minimum Qualifications/Requirements |
- Basic knowledge
and skills in all aspects of nursing, psychiatric disorder, treatment
of mental illness conditions, general pharmacology, medical record keeping,
infection control, laboratory results, and the operation of routine
medical equipment.
- Advanced knowledge
and skills in psychotropic medications, medication administration, crisis
intervention and emergency care, nursing assessment, and patient education.
- Agreement to
use personal transportation in work capacity.

- Licensed in the
State of Indiana as a Registered nurse or a Licensed Practical Nurse.
Registered Nurse preferred.
- Valid Indiana
driver’s license.
- Current CPR certification.
(Re-certification annually)
- Current First
Aid certification. (Re-certification every 3 years)
- Current CPI certification.
(Re-certification annually)
- Maintain TB Certification
While performing
the essential duties of this job, the employee may be required to climb
and/or lift.
Nurses work in various
community settings with limited on-site supervision. Office and program
areas may not be air conditioned and could become overly warm in summer
months. Many program activities are conducted in community settings away
from Hamilton Center, Inc. premises. Work conditions, such as cleanliness,
comfort and safety may vary widely. Exposure to all types of weather elements
is likely. Staff may be scheduled to work evenings and/or weekends. Safety
management and crisis intervention decisions and actions are dependent
on staff present. Management of emergencies is dependent on staff and/or
outside emergency services. A crisis could arise which may be stress producing
and/or exhausting.
- Satisfactory
reference and background investigation checks
- Completion of
tuberculin screening no later than three days prior to first work day
of employment and annually thereafter.
- Completion of
a pre-employment drug screening and completion of post-employment drug
or alcohol tests upon reasonable suspicion of use.
- Completion of
Center-wide new employee orientation and ALL required paperwork prior
to reporting for work.
- Completion of
Total Quality Management training within first six months of employment.
- Attendance at
all mandatory staff development and training.
- Successful completion
of a six month on the job orientation period.
- Participation
in payroll electronic deposit.
- Adherence to
Compliance Program Plan.
